We find the stud or solid backing and anchor directly when it lines up, we drill tile with carbide tip bits at low speed so nothing cracks, and where a stud is not in the right place we use toggle anchors rated at 300 lbs per anchor with two anchors per bar end.
We never use standard drywall anchors, suction cup bars, or towel bar hardware for a grab bar meant to bear body weight. Every bar is load tested at 250 lbs before we leave, and any bar that shifts under the test gets re anchored on the spot at no charge.
